Substance use exists on a spectrum, and so does the right level of care. For some people, outpatient therapy is enough support to make real progress. For others, a more structured program is a safer and more effective place to start. This page can help you get a general sense of where you might fall, though the best way to know for sure is to talk it through with our team.

When Outpatient Therapy May Be a Good Fit

Standard outpatient therapy (one session a week, built around your regular life) tends to work well for people who:
  • Are in remission or have sustained a period of stability in their recovery
  • Have occasional or sporadic use rather than daily or escalating use
  • Are able to maintain work, school, and family responsibilities
  • Do not have significant withdrawal risk if they stop or cut back
  • Have a stable, supportive home environment
  • Are motivated to engage in treatment and follow through between sessions
Outpatient therapy at Breakthroughs can support ongoing recovery, help address the anxiety, depression, trauma, or relationship issues that often accompany substance use, and provide a steady source of accountability and support.

When a Higher Level of Care May Be Needed

Some situations call for more structure and clinical support than weekly outpatient therapy can provide. This may include:
  • Daily or near-daily use, or use that has been escalating
  • Binge drinking 2-3 times a week
  • Withdrawal symptoms, or a history of withdrawal complications
  • Previous attempts at outpatient treatment that haven’t held
  • Difficulty staying safe or functioning day to day because of use
  • Co-occurring mental health symptoms that are also intensifying
  • A home environment that makes it hard to stay stable without daily support
In situations like these, a program with more structure (such as an Intensive Outpatient Program (IOP) or Partial Hospitalization Program (PHP)) is often a better and safer starting point. These programs offer more frequent, more structured treatment than traditional outpatient therapy while still allowing clients to live at home.

For clients who need this level of support, Breakthroughs is connected to Sophros Recovery, our sister organization offering IOP and PHP for substance use and co-occurring mental health needs. Our team can help make that connection, and many clients later step down to outpatient therapy at Breakthroughs once they’ve completed a higher level of care.
