For Existing Clients
This page has quick answers for common questions about scheduling, your therapist, and getting support for other people in your life.
Scheduling, Rescheduling, or Canceling an Appointment
The fastest way to manage your appointments is through your client portal.
- Log on to your client portal from the main page at jaxtherapists.com.
- From there, you can view upcoming appointments, request a new appointment, or reschedule and cancel existing ones.
If you run into an issue with the portal, you can also text us at (904) 419-6102 or call our office during business hours, Monday through Friday, 9:00 AM to 5:00 PM.
Please note: text and portal messages are not monitored in real time. If you are experiencing a mental health emergency, do not wait for a reply.
If You're Having an Emergency
Breakthroughs is an outpatient therapy practice, not an emergency or crisis service. If you or someone you love is in immediate danger, please use one of the resources below rather than waiting for a call or text back from our office.
- Call 911 if there is immediate danger to your safety or someone else’s.
- Go to the nearest emergency room for a medical or psychiatric emergency.
- Call or text 988 to reach the Suicide & Crisis Lifeline, available 24/7 for support during a mental health or emotional crisis.
If you’re not sure whether what you’re experiencing is an emergency, it’s always okay to use one of these resources. They’re there to help you figure that out too.
If Your Therapist Isn't the Right Fit
Therapy works best when you feel comfortable with your therapist, and it’s normal for that fit not to be right every time. If something isn’t clicking — whether it’s approach, personality, scheduling, or anything else — let us know. You don’t need to explain in detail or worry about hurting anyone’s feelings.
Text or call our office at (904) 419-6102 and ask to switch therapists. Our team will help match you with someone who may be a better fit based on what you’re looking for and who has availability.
Getting Help for a Family Member
If someone else in your life could use support, we’re glad to help with that too. Depending on what’s going on, that might look like:
- Setting them up with their own individual therapist at Breakthroughs
- Starting family counseling together, with you and your current therapist or a different clinician
- Connecting them with Sophros Recovery if they need a higher level of care, such as IOP or PHP, for substance use or mental health needs
Text or call (904) 419-6102 and let us know who needs support and what’s going on. We can talk through the options together.
A couple of resources that may help:
- 988 Suicide & Crisis Lifeline — call or text 988, available 24/7
- SAMHSA National Helpline — 1-800-662-4357, free and confidential support for individuals and families facing mental health or substance use concerns
